Missing Truck Items Reward Points Form
Truck deliveries are a vital part of a restaurants ability to operate. It’s important that we check in every item that we ordered so we are not over charged for items not received. While checking in the truck order, if any item is missing, the employee may fill out a form to be eligible for a bonus within 4 hours of the delivery. Only one form per truck order per restaurant will be considered. Please allow 1-2 weeks for the reward points processing. The employee, GM or manager may fill out the form. Additionally the reward points can be split in the event that multiple employees spot the missing product. Missing items do occur time to time. While our distributor aims to have no errors mistakes happen.
Calling in cases, you will be eligilbe:
- 500 points per case missing up to 10 cases
YOU MUST CALL MISSING ITEMS INTO DC WITHIN 4 HOURS OF DELIVERY TO GET CREDIT.
